Information in a property request

The form collects property and ownership details, legal owner or entity name where applicable, email and phone contact information, and your communication consent. Commercial requests may include construction, roof, protection, condition, insurance history, and requested coverage details. Building owners may also provide claims history. After at least one year of ownership, the form asks whether insurer-issued loss runs are available; documents are not uploaded through this form. Optional comments let you add context. Payment information, Social Security numbers, passwords, and medical information are not requested.

Where a submission goes

Jotform hosts the form and stores submitted requests in the connected intake account. A notification containing the request is sent to the Insured ASAP commercial intake team for review and follow-up. Sending a request does not create an insurance policy.

Use and retention

Information is used to understand the property, determine whether the request can be handled, and contact you about insurance options or additional details. Information needed to obtain coverage may be shared with appropriate insurance providers during that process. Submitted information may remain in the intake account and notification mailboxes; closing this website does not delete a submitted request.

Phone calls and text messages

The online form requires an unchecked acknowledgement giving Vacant.insure permission to contact you by phone, email, and text about your quote request and related follow-up. Providing a phone number alone does not record that acknowledgement. Consent is not required to purchase insurance and may be withdrawn at any time.

Message frequency varies, and message and data rates may apply. Reply STOP to opt out of texts or HELP for assistance. You may withdraw call permission by telling the caller or contacting the intake team about your request. Mobile opt-in information and text-message consent are not shared with third parties or affiliates for marketing or promotional purposes. Service providers may process that information solely to deliver the requested communications.

Website chat

Podium provides the website chat widget. If you use it, the name, contact details, and messages you provide are processed through Podium so we can respond. Podium may also process browser, device, and website interaction information to provide its service. See Podium’s privacy policy for its practices. Chat messages do not bind, change, or cancel insurance.

Website and form providers

The website host and Jotform may process technical information, including network and browser information, to deliver their services and help prevent abuse. The site includes the Jotform quote form and Podium chat widget. Their services may use their own cookies or similar technologies. Jotform’s embedded service has its own privacy practices, described in its privacy policy.

Information in links

Public page links use clean paths. A selected property category, state, or situation may be held temporarily in your browser session and passed to the embedded form. Your name, street address, email, phone number, parcel identifier, and message are not stored in that navigation context or placed in page links.

Questions and corrections

The request form is the current contact route for questions about a submitted request or corrections to your information. Identify the request without including sensitive documents or account passwords. Requests to access or delete information are reviewed subject to applicable recordkeeping requirements. The planned public mailbox is quote@vacant.insure and is still being set up.
